Fall Sports Announcements
The Athletic Director must clear you before you can participate. In order to be cleared you must:
- Complete the On-Line Registration process- see below
- Have a current physical on file with the health office. A physical is considered current if the date of exam is within 13 months of the first date of practice.
- For incoming sophomores, juniors and seniors, you must have a passing grade in at least 5 classes in your final F1 grade. If you only passed 4 classes you may apply for an Eligibility Waiver. Contact the Athletic Director for more information.
- All incoming freshman, juniors and any student that did not take an ImPACT test at John Stark last year regardless of sport played must take a concussion baseline test. In order to take the test the on-line registration process must have been completed by a parent/guardian.
On-Line Registration Process: The John Stark Athletic Department has partnered with a company called Family ID that will allow parents to complete the required athletic handbook forms on-line. The on-line process will be active starting July 1. To start the process of registering your son and/or daughter to play a fall sports please go to the following website https://www.familyid.com/john-stark-regional-high-school. Then you will need to create a log in by entering in an email address and password. An email will be sent to the address provided to finish the log in creation process. Once that is done you will be ready to start registering your child(ren) for fall sports. If you have any problems during the process you can contact the Family ID customer support at (888) 800-5583 x1 or the John Stark Athletic Department at 529-5301. Those that completed the process in previous seasons will just need to log in using the email address and password you used in the past.
If you don’t have Internet access or a computer at home, a parent may come in at one of the times set-up for baseline concussion testing to complete the forms using a school computer.
Baseline Testing: All incoming freshman, juniors and new students to John Stark regardless of sport played must take a concussion baseline test in order to be cleared to participate. Parents must complete the on-line registration process before an athlete can take the baseline test. It is also suggested for incoming freshman, new students and those that had a physical exam during the summer bring a copy of the physical exam form with you. Test will start on the time as listed below. Please arrive at least 10 minutes prior to be checked in. Any athlete arriving after the test has started may not be allowed to be tested during that session.
Dates Concussion Baseline Test will be given are as follows:
Thursday, August 8- Testing will start at 9:00 am, 10:00 am and 11:00 am
Monday, August 12- Testing will start at 3:30 pm, 4:30 pm and 5:30 pm
Monday, August 19- Testing will start at 8:00 am, 9:00 am, 10:00 am, 1:00 pm and 2:00 pm
